We understand that in the ever changing world of insurance it can be difficult to understand your insurance benefits. Our experienced staff will be happy to explain and obtain insurance benefits for you at the time of your visit. In order for our staff to obtain complete benefits we request that you bring BOTH medical and dental insurance cards with you at the time of your appointment.

We do request payment on the day services are performed.
For insurance companies we are contracted with, once we are able to obtain complete insurance benefit information, our office will work with this information to get an estimate of your responsibility for services being rendered. Please understand that this information is based on the information provided by your insurance carrier. Payment for services are ultimately the patients responsibility. Any estimated patient portion will be collected from you at the time services are rendered.
Please understand that we do not know actual insurance coverage until we receive payment from your insurance company. Once we receive payment if you have paid us too much we will send you a check for reimbursement. If you have paid us too little you will receive a bill from our office. If you have any questions regarding insurance coverage or a bill from our office please contact us and we will be happy to help you.
It is our office policy to collect in full for services for insurance companies we are not contracted with. You will then receive reimbursement directly from your insurance company.
Please note that if you are being seen for something medical in nature (example: biopsy, TMJ) and your medical insurance requires a referral from your primary care physician to see a specialist we do ask that you request the referral prior to being seen in our office.
